I have characters of both factions on the Alterac Mountains server, and when I try to enter Vault of Archavon on 2 different faction toons parked outside, neither can enter. That is to say, if I log on my Alliance character, I’m told it’s Horde controlled. If I log over 20 seconds later to my Horde character on the same server in the same location, I’m told it’s Alliance controlled. How exactly does this work?

The server you login to when it comes to old content is not guaranteed to be your cluster or server. If you turn off warmode you have a higher chance of it being the same, but the same reason you see different people on different servers in the world is why you’ll see different vaults. Try server hopping or changing WM to non or the opposite.
