There are a few ore nodes in westfall that are unreachable. The ones I have found are near mines, it appears that they should have spawned in the mine, but positioning was off and they spawned outside the wall. This causes them to show up in tracking as if they are on the surface, and when you go to their location the can not be seen.
I submitted a bug report in game for the first one I found, after finding 2 more I thought I should post it here too.

I remember this from vanilla, the worst offenders were always around deadmines. The minimap would show the vein as being outside when it was really underground.
I have quested through westfall, and it is as a i remember, ie it is a feature and not a bug.
The issue is that when you go to that spot underground, you the node isn’t there.
I can confirm this. There are a couple in the dagger hills, but one just south of Gold Coast Quarry. Especially in the latter case, it is easy to confirm that the node is neither in the cave, nor on the bluffs. Wouldn’t the end result of this bug be fewer nodes to mine? This is especially annoying as most of the offenders are tin nodes, which are hard to come by anyway.
Theres also one in Duskwood on the mine where the Ogre monacle quest mob is.
Just to the right of the mine outside but inside of a rock type thing that looks like it was added at a later date as it does not fit with the scheme of the area.
I know blizzard said no chances but this is one thing I would think can/should be fixed.
