Unable to leave combat

My DH was questing in Hallowfall and got stuck in combat. I’ve had it happen a few times on alts doing the Nightfall scenario, but on them, logging in and out would fix it. However, leaving the game did not work for my DH, nor did hearthing. Everytime I log back into her, I’m still in combat and I can’t mount or take a taxi, making it essentially impossible to play the character.

Does anyone have any workarounds I can try to fix it?

I’ve had this happen multiple times in any part of the map that has Nightfall associated content. Rogue’s vanish won’t break it, ground pounding your way to the Weaver’s Lair in Azj-Kahet and taking the portal to Dornogal sometimes (only sometimes) works. The only thing I have found that will sure-fire break it is to find some way to die, usually jumping off a cliff - of which, there are plenty in Hallowfall.
