I was in the process of looting Swarming Cleaver of Ka’roz and before i hit the “accept” button that popped up, i moved my character, which made me drop it. The treasure then disappeared, so i was unable to click it again. I opened an in game ticket and GM Onqoopneok told me I should submit a bug report because the item neither showed up in my in game mail or item restoration. Hopefully I could get the item restored to the character.

Same thing happened to me
Same thing here. Looted the Swarming Cleaver of Ka’roz, got the Achievement “Lost and Found” but didn’t get the item in my bags, item despawned from the world. GM Roodhephet told me to make a bug report when I made a ticket.
It’s such a nice sword, I was so excited to get it

Received XP and achievement credit, however didn’t loot the item because I moved. Now can’t loot the item again. Tried the command to see if I have looted it and it says I’ve completed it, but never had the item. My item was the Bloodstained Chitin Fragment.

Same thing happened to me when I went for the Swarming Cleaver, even had to double check because I saw the experience in my chat window. If it wasn’t for the fact it’s an insanely farmed item I’d just try on another character.
