I’m sorry, is nobody going to talk about how the new transmog system has been broken since they rolled it out? And it’s not getting better with time, it’s getting worse. The tuesday reset JUST came too and nothing was done. Why? Go shift through your outfits and you’ll see the problem. Or is it just me??? Different clothes keep appearing on my sets, nothing stays the same, the entire thing is just buggy and awful I can’t get a single outfit to stick. Parts of one outfit will suddenly show up on another.

I have a problem where some characters have no problem with transmog while others just don’t work. On those you click the icon, your 'toon spins around and nothing changes.
It may have to do with having an outfit that is too low level but regardless if it is not going to work you should get an error message saying why it is not working.
I’m glad I have not run into any issues with the new system, it’s amazing. Sorry to hear that.
If you mean on the character selection screen, its applying base color of that item if there are multiple versions. Logging into the character goes back to whatever they have applied, its just a visual thing.
When that happens I usually have to select the outfit beneath it and then select the outfit directly above it several times until it displays the correct transmog. Eventually it works, but it may break again if you pick outfits that are farther apart from each other on the menu.
The transmogs themselves aren’t actually affected, so you can still switch between them just fine despite it.

i am finding that if i upgrade an item, it undoes my transmog. i have to switch to a diff spec then go back in order to see the right set. if i hop up on the xmog mount, it will not let me hit the apply either. it doesn’t LOOK right but according to the xmog mount, its the right one. 
oh try that, if you are having weird issues where you suddenly lose your transmog— switch to a diff spec then go back. so far, its been fixing the issue, but still … ugh, blizzzzzz.
Go to the situations tab for every outfit slot you have assigned a transmog set to and uncheck ALL the situations… then if you still have an issue, report back about exactly what you did, what you expected to happen, and what actually happened.
I’m talking about the box that says “Situations Enabled” below on the left-side…
I saw an earlier post about someone having their transmog randomly change when in their House and they had multiple outfit slots with Situations Enabled / Locations / House but they had no clue they had enabled that. Turning it off fixed their issue.
It’s great if you want automatic outfit switching (I have that on for House, non-House Rest Zone, and “all other cases”), but it can be confusing if you don’t understand how easy it is to create conflicting situation assignments.

It’s worse when you pay the gold to update your weapon mog and then for some reason it just doesn’t work? I think it’s getting confused because I have a 2 handed weapon for dps spec and a shield + one handed weapon for my tank spec? I’ll try removing situations completely to see what happens, right now I think it’s set to the default of Situations Enabled and everything is listed as All.
It’s really annoying that Outfits can’t be reorganized, too. I want my third slot to be moved up to my top main slot. It’s only going to get more annoying the more slots I buy since I like ordering things from most liked to least liked. Maybe I’m just a weirdo. 
I didn’t know about this until a friend told me (despite playing with Xmog 2.0 on Beta quite a bit), but if you click the weapon or shield/off-hand there’s a fly-out menu to let you pick the transmog for 2-Hander -AND- 1-Hander+Shield in the same Outfit Slot.
Due to lack of knowledge, I had used two Outfit Slots, one for Tank and one for DPS to deal with this and use situations based on specialization (Prot or Ret respectively), but turns out that was entirely unecessary.
I don’t think that’s you being a weirdo… several other players have asked for that same feature; to be able to re-order the Outfit Slots like we can re-order our characters on the login screen (so the top-preference ones are at the in the Campground, while others are in a non-Camp list ordered how we wanted them). I want that for Outfit Slots too, but not going to lose sleep over it (it’s a want for me, not a need).

This is par for the course with these new bugs but I noticed that I also had a recolor equipped that I don’t even have learned! How is this even possible? I can understand it having trouble when the item has the same name between LFR/Normal/Heroic/Mythic but it showed my character using the regular PvP version of the set without even owning it.

Yeah. Unfortunately, this has been happening to me too. It transmogs my toons just fine, but when I want to sift through them to view their appearance (my custom made sets, not to be confused with the custom sets tab), it gives me different sets and mismatched items and bugs out.
I’d put in a bug report in the bug forums to let them know if I were you. I’m about to head out of the house at the moment, so I’m not able to do it right this very minute.
Edit: OP, you can also edit this thread and just move it to the bugs section so they can see this issue and try to fix it.

If so, dump it…its horrible with this new system…literally showing TONS of incomplete sets and missing appearances
I’ve only had problems with Artifact Weapon appearances (specifically Off-hand and Shields) either not showing up on login (Skull of the Man’ari) or showing the equipped item instead when using a 2H weapon (Fist of Ra-den).
