Today I ran a layer 3 Mort’regar and after exiting (and getting the achieve for running a layer there) I went back to the door and the tracker does not show completion, of any layers. I also did not get the soul ash from the completion. I had completed layer 3 of the Fracture Chambers previously, so I jumped directly to layer 3 Mort’regar.
