Looks like Dark Heart or something else recently broke it? On wowhead seems like people have done it as recently as March 31st.

Yeah - something seems wrong, killed four different elites each with 10 stacks of the buff - no achievement.
Having the same issue with Avenge Me and Rampage.

Yep it seems broken, I couldn’t get it to trigger either. Hopefully it can be fixed soon.

Broken for me too. This was the last piece I needed for Tower Ranger.

Same here. Tried it last night several times.
Tried it with Avenger (empowered) at level 16, went back to level 11 for Avenger, did both with war mode on and off, nothing worked to get the chiev to kick in.
Even thought maybe killing too fast, then i got one to 10, backed my self into a corner and let it attack me for 3 minutes and then i killed it. Nothing.
This is last I needed for Tower Ranger too.

Same! WM on or off, Add ons on or off. Empowered Avenger or not. No achievement. 
Same here, this and Rampage are broken for me. The last things I need to complete the Shadowlands Meta achievement. I sent a ticket to Blizzard about it but we need more so they can look into this. Who ever has this achievement and Rampage bugged be sure to let them know and send a bug report.
If they can’t fix this in due time they need to just remove it from the prerequisites (along with the other achievement causing issues) so folks can get the meta they rightfully worked on and earned.
Went around and around with Customer Service all this week, about this and “Taking from the Earth” achievements. Both of these minor MINOR sub-achievements prevent players from completing “A World Awoken” and “Back from Beyond” and earning those mounts. C/S unfortunately left me with the impression that the Devs don’t care - they’re concentrating on their new cash cow (War Within Beta).
We’re going to need to get a lot louder if we want them to pay attention.
I am having the same problem as well. Kill multiple elites with a 10 stack and did not get the achievement.
Ditto - this is bugged. Appears to complete, but achivement doesn’t register.
Damn, came here hoping it was fixed yet. Literally one of the last four things I need to complete this meta.
Me and a buddy of mine had the same issue last night we did the Achievement like 10 plus times. No Achievement still
Bugged for me too, this is seriously rediculous.
Sigh, still not able to complete this… Also can’t seem to get reknown rewards for the niffen…mail snail doesn’t appear. Trying to get things done with the insane drop rate is one thing (maldraxus backpack from the slime debacle) but now you’re just trolling us bliz. I get it, its a time when you’re worried about people stepping away for a bit so why fix things that complete things for people but this is starting to just get plain stupid.
Still bugged. Even if you have already completed it, it’s no longer counting towards the meta.
