Has Throw Glaive always only been usable while in Combat or is that something introduced with the 10.0 patch?

I just re read your post actually. Throw Glaive isn’t only usable in combat though, it’s always been usable outside combat like you said

It’s always been usable out of combat.
There’s a talent in the spec tree called Furious Throws that adds a 25 fury cost to Throw Glaive.
I specc’d out of it because it felt weird not to be able to use it as an opener.
