If you burn down the first part of the encounter for Mindbender Ghur'sha too quickly you end up trapped in that room. The gate won't release and you're left alone with Erunak Stonespeaker. It's gotta be a bug. Mindbender Ghur'sha isn't even in the room and cannot be killed to trigger the release of the gate.
Just happened to me in a Timewalking group. Didn't even dps too fast either. Second phase just didn't occur.
