I think it was the reward for freezing the pipes on the big pump in ringing deeps, gave me a one hand battleaxe for my prot pally.
After riding the drill to Undermine, it POOF vanished in to thin air.
Apparently I am not allowed to have it.
I would make a bug report except i never expect to see the thing ever again, so I am just letting Y’all know this happened.
Are you sure you didn’t just see the notification about unlocking the appearance? Because you get all possible appearances of items a quest can reward in addition to the actual item you receive.
But also there doesn’t seem to be any axe by that name on wowhead, so I dunno. Maybe you just got the name wrong.

battle axe was definitely the suffix,
Update; the world quest to close the kobold tunnels dropped a nice 1h mace to replace the axe I lost.
It’s like the game knows I needed a good 1 hand.
