The Lich King is a metaphysical entity - the one who carries its mantle is just a throttle. Without a figurehead the essence of undeath would be free to bleed endlessly from the Frozen Throne. Bolvar acts as a spigot controlling the flow of foul energies from whatever realm the Nathrezim tethered ours to (presumably the source realm of undeath.)
