In Sanctum of Domination, the final boss Sylvanas is unkillable. She can be damaged but no phases happen and she just stands there.

She has been working with the shadopan and is now so zen that even an entire raid cannot break through her meditation.
We are experiencing the same problem. We are currently standing there around here just hitting her with auto attacks slowly and she’s not phasing.
