Currently, while doing PVP content in the expansion, I am still getting the item Shred of Insanity. I believe during the first week of the pre-patch event I had finally completed the quest for 200 shreds for the crafting component item, Dreadful Void Focus. Just about every battleground I completed after this quest completion still provided the Shred of Insanity.
This at first was a little funny. Now coming into the second week of the new expansion, and still receiving this item that forces me to confirm deletion is driving me insane.

Same thing is happening to me. I thought it was because I dropped the quest " Maintaining Focus ", and so I tried to pick it up again so I could turn in all these damn shreds. I can’t seem to get Wrathion to give me the quest again, and I keep getting these drops in every BG.

Same thing here. I abandoned all my old BFA quests and I’m still getting them after every BG.
Same here. – Only really noted it very recently and hadn’t pinpointed PvP as the catalyst, but that’s very likely the case.
Guessing I abandoned the applicable quest chain when I cleared out my log at the start of SL.
Just wanted to add I am also getting these, not PvP though, I’m getting them from WQ’s.
Just got one from an Ardenweald WQ, I got 3 yesterday and 3 the day before that.
I recently started BGing again though, so it’s possible that PvP is triggering the bug.
Yea I’m having this bug too. I have looted like 6 SoI this week from doing pvp for the first time in shadowlands. Surprised this bug is still around looking at your time stamps of December.

Anyone have any word on this? STILL GETTING these Shards, a full expansion later. Super annoying.
Maybe we’re all just a … shred insane. 
I am still getting these from BGs. Has anyone figured out how to stop it? It’s been 2 expansions. Come on
