Is it better/easier/more efficient to start one on a pve or pvp realm?
overpopulated or underpopulated?
Wotlk or Era?
I want to make a medium to small sized guild, not overly large or zerg-ish.
Any other suggestions would be highly welcome for a new guild leader!

It’s an incredible amount of work to be successful. But you should do whatever you want. Building a community is no small feat. Sustaining it is even more difficult.
You should probably go on a realm that is high pop but not full and not low. Medium maybe.

As someone who has started many raiding guilds over the years on different servers, I will answer your questions to try and help ya out from my own experiences.
First, realm type doesn’t matter; population, age of the server, and phase of the expansion the server is in matters.
Which leads to the next point. If you don’t care about which version you are playing, you’d be better off making your guild on a wotlk server. Higher population and earlier phase than era servers.
Keep in mind when I say population, that higher population means a bigger pool of people to recruit from. But it also means more competition for your guild, and the hardest part of creating a guild is the first few months when you don’t have much to offer people in terms of content clearing, people to chill with, organization and history, all those kinds of things an established guild will already have. So pick your poison there. I would recommend choosing either a high pop server or a medium pop server. Something like, if alliance side, Benediction or Pagle for a high pop choice, Atiesh or Old Blanchy for medium pop. Don’t choose a low pop server because chances are most of the server is already in the same few guilds and people aren’t likely to want to switch out to a brand new guild.
The biggest tip I got for you man is don’t try and do this alone, you are gonna burn out and not have fun. If you want to start a guild to actually build up to eventually running end game content with, you are going to need help. Bring friends and fellow gamers you get along with and trust and get them to help you. Divying out responsibilities in guild leadership is one of the most important things you can do as a guild leader. This is a lesson I’ve learned the hard way over the years and am still improving on myself :).

I wouldn’t recommend starting a guild unless you have a clear vision and goals. Where you can find like-minded people to make those a success.
Honestly reading your OP you don’t seem to really have any plan. This isn’t the kind of thing to go into that way.
Why do you want to start a guild? Creating and running a guild right is a lot of work.
