When a macro is titled with anything but has the following commands, the Macro will not show Macro Text on Action Bars (no addon for bars installed):
#showtooltip
/cast Starfire
/cast Warrior of Elune
But if you swap WoE with Starfire the Macro Text will show on Action Bar:
#showtooltip
/cast Warrior of Elune
/cast Starfire
One needs a target and the other one does not, or self.
The macro itself works. It’s the macro text that doesn’t populate on the Action Bar. Even in the Macro UI it shows Macro Text for the button.

Cast … Spell one … Needs a target to show tool tip.
Cast … Spell two has a target ; you and shows the tool tip.
Cast … Two shows you as target.
Cast … One has no target. Does not show tool tip.
That is why.
The first condition even if false is still … Show tool tip, but can’t because it not true condition.
Macros in retail goes top down.
Macros in classic does not.
It is confusing.
If you have a target the tool tip if cast is first it will show. If not it will not show.
The macro needs a condition.
/Cast [ harm]starfal;[help]heal

This isn’t a tooltip issue or targets. The name of the Macro isn’t showing on the Action Bar if Starfire is the first spell or Tooltip spell. When any other spell is the tooltip it’ll show the Macro Title/Name in the Action Bar.
I think it’s because of the charges to engage Eclipse showing at the bottom right of the spell icon.
