I remember when they introduced it in Phase 3 and it had a 0.5% drop chance. Then they introduced the updated portal system in phase 4 and it was just as annoying to farm. I have now spent 2 months looking for it, saw 1 green portal total (if you know you know), and apparently this level 50 trinket is now best in slot?

It most certainly is not BiS. Not even top 10 trinkets this phase. Whoever told you it is BiS or what website is wrong.
As for portals they have set spawn locations in specific zones and the specific item is from Fel Scars in the Blasted Lands, Feralas, and Azshara and you cannot use the portal tuner on the portals or else the loot table changes.
25% more damage on an endless, infinitely-scaling Agony? That sounds like Bis to me.
The loot table changes, yes, but you have to overcharge the portals to have a chance at the green portal, which has a chance itself to have the trinket. Layers upon layers of rng that monopolize your time. That doesn’t sound like good game design to me.
Oh your talking about for Phase 7 with 6 piece tier set not the current phase.
Guess it will depends how the tests go and if it stacks or doesn’t stack with Amplified Curse (50% increased damage).
You don’t. Regular Feralas, Blasted Lands, and Ashara portals drop the trinket. Feralas portals are the easiest to find as they are in a small area. Make a macro /tar Fel Scar and spam it while riding around in the small area. 1 Fel portal will always be up.

From my understanding Overcharged Portals do not have a chance to drop it.
