I have done 3 heroic dungeons today. Here’s what I got:
Spires of Ascension - 4th boss - Back
Mists of Tirna Scithe - 3rd boss - Shoulders
Necrotic Wake - got nothing
Shouldn’t I be getting at least one more item at least once? I will continue logging my dungeon drops.
I also got the Hunger for Knowledge Mythic achievement for Mists (last boss) in Heroic mode. I have never done a Shadowlands Mythic on this character.

Sign of the Warrior does not drop an additional item per player, it adds one additional item to the personal loot pool.
As far as the mythic achievement, I would recommend reporting it in the Bug Report forums or via the in-game support tool for QA to investigate.
So this week my first mythic dropped 2 items for me. Since then i have done every mythic 0 and 2 mythic 2+ and had NOTHING else drop!
That is the fickle nature of personal loot RNG, unfortunately-- some weeks you get lucky and are overwhelmingly showered in drops. Other weeks, total droughts.
However, if you feel that the situation is a bug, you’ll want to report it in the aptly-named Bug Report forum for QA to investigate and escalate if their findings warrant it.
