How am I supposed to hold my mouse with my index finger on the J key? 
Personally, I hate the ESDF setup because I hold the side of my keyboard with my left pinky, and don’t actually use it. I do the same thing with my right pinky to stabilize my mouse.
But I don’t actually move around with them anyway. I use my mouse for movement, Q and E to strafe, and only ever use WASD to bump my movement a hair for a specific reason.

I use 123456789-= for my first bar
I use !@#$%^&*()_+ for my second bar
and then my third bar uses the g-shift on my G600 gaming mouse.
I will also use q e and r for other buttons respectively.
For me the more buttons I have the better. If you have the money get an MMO mouse and you will not regret it. It lets me keep my left hand free to move my character most of the time will letting me quickly use my skill across three hotbars. The best part is the reprogrammable G-shift is set to ctrl by default so I no longer have to reach my pink all the way down to use any skills on my third hot bar.
