I have. It’s what got me into WoW. Lorewise, it was forged by the Lich King according to dialogue. Later material, particularly ancilliary from WoW, would claim it was the Dreadlords who created it. Then Shadowlands claimed it was forged by the Runecarver(AKA The Primus) in shadowlands.
Locations and how they look never looked too consistent between Warcraft 3 and WoW usually, but they don’t usually stray that far. The only noticeable exception was the Black Citadel retcon from Warcraft 3 to Burning Crusader. The Black Temple and the Black Citadel were supposed to be one in the same in Warcraft 3, but WoW retconned them to be separate locations. It’s particularly noticeable when you compare the siege to the Citadel, where Magtheridon rules Outland, in Warcraft 3 and then you compare its despiction in WoW (like the patch 2.1 trailer and the questlines ingame), where Magtheridon ruled from the Black Temple in Shadowmoon Valley before Illidan usurped control.
First time it was spoken, it was Tichondrius who claimed The Lich King forged the blade:

The next time it was discussed was in the Arthas: Rise of the Lich King novel (Page 204 according to the warcraft wiki), where it was stated the Dreadlords forged it.
Then World of Warcraft: Chronicle Volume 3 Page 53 discusses it again, detailing how the Nathrezim forged it and the precautions they took to make sure the blade didn’t claim their souls ( presumably to accomodate the fact WOTLK revealed Mal’ganis survived being struck by the Blade).
The Runecarver creating the blade is a retcon first introduced in the Shadowlands expansion, something even the Warcraft wiki implicitly acknowledges as a retcon in the Trivia section of Frostmourne.
