Granted back in legion I did the mage tower and opened up the epic skin for the scythe of elune now I’m going for the red version on heroic kiljaden do I have to have it equipped to earn it or is my current weapon fine.
No, just kill KJ on Heroic and it’ll unlock after a relog or two.

In my experience, the skins unlock based on the criteria - it isn’t attached to the actual weapon itself, you can have anything equipped.

No. So long as you do it on the character that has the base version unlocked, it doesn’t matter what you’ve got equipped.
