Same item from m+ chest four weeks in a row

I’ve received the same item in my keystone chest four weeks in a row now.

I’m wondering if something is broken with the algorithm? Like of all the loot in all the dungeons, I get the same one four weeks in a row? It seems a little strange. Should I file a ticket? There’s bad luck, and then there’s this.
