Once you pick up Baines weapon, you’re supposed to slay Tower Inquisitor. The inquisitor never spawns.

Exact same symptoms for my paladin. Relogging, dropping and reacquiring quest etc. doesn’t help.
I’m in exactly the same situation. Dropping the quest and trying again doesn’t help. Exited the game, turned off all addons - nothing gets it to work.

I am having the same problem. Second bug this week in the Maw that has completely stopped my ability to progress. This is getting very old. I have a lot of toons to level and I can’t keep running into these kinds of roadblocks. These are the types of things that makes me quit playing.

Same problem here, dropped quest and tried again but Tower Inquisitor did not spawn either time. As Atrocituss said above, it’s put a complete stop on my ability to progress further in the campaign. Frustrating and tiresome…

I ended up going back to Soulbinder Daelia and just skipped ahead to the soulbind portion.
Having this same issue. Opened a ticket, directed to Wowhead and these forums. I’ve re-logged, exited the game completely, disabled all add-ons, abandoned, and picked up the quest again. NOTHING. Tower Inquisitor corpses all around me but NONE to actually kill for the quest objective.

Same issue I’m having.
ok I found the fix, it worked for me at least.
go to the group finder and go to the custom groups, from there join a random group (not a raid) once invited it should phase you into a different version of the maw, and the mob will appear. the fact that this was a similar problem back in November of last year with the same quest shows the level of competence in this games development and QA.

Confirmed. At first, I was standing in the area where he should spawn and was joining groups with no luck. Then by chance, I joined another group and walked in/out of the area he spawns and it worked. I killed him before leaving the group just to be safe.

THIS WORKED. went back to pick up the quest after abandoning it. walked back to place the cypher but joined a custom PVE group in the maw first. placed the cypher, grabbed Baine’s mace and the Inquisitor APPEARED. killed it, looted the body, dropped the group and was able to turn it at Ve’nari. JFC. this was brutal.

Rule 3: Trust is earned tower inquisitor – never spawns for me. Tried several things including abandoning the quest, and then joining a custom pve group before placing the beacon. Tried logging out and logging in. Dude does not spawn for me.

Yup, broken… again. Can’t get the first step completed - Signaling Beacon Placed.
No item in inventory, nothing to click.
Unfortunately, even trying the trick of joining a custom group of a friend on an RP realm (confirmed it phased me) didn’t resolve it.
Wonderful, can’t progress in this questline further.

Quest: Rule 3: Trust is Earned
Step: Signaling Beacon Placed
After accepting the quest, map pin shown for a sec but suddenly disappeared,
and quest object didn’t spawn at target location.
