I have this item on two of my tanks, one a DK the other Warrior. Today I noticed on my DK that it proc’d exactly once in an entire dungeon run. Fast forward to later during a razagath kill on my warrior and the same issue happened, proc’d once the entire fight.
Not sure if its a bug, but the RPPM of the trinket is really low. In case you used logged the fights, you should check for multiple proc possibilites. It can proc a single target dmg effect (fire shot), aoe dmg (lobbing fire nova), a single target heal (curing whiff), a multi target heal (mending breath), a haste buff (under red wings) and a crit buff (sleepy ruby warmth).
However, even if its not a bug, the trinket iself is very inconsistent. Training one of the abilities is supposed to increase the chance of ability to proc. They should either increase the RPPM or increase the chance of the trained ability to proc.
In both instances according to details, during a 40 minute dungeon run (and nearly 15 minute raz kill) on two different characters both with ilvl 392 Ruby Whelp Shells, the only ability associated with the trinket that proc’d was fire shot, once. And only for around 70k damage or so if memory serves me correct.
I do not believe that that can be by design. It wouldn’t make any sense for a trinket to proc so little and yield such a small result in either of those time frames.
Your post lacks enough details to know if you are using it correctly.
Have you “trained” the trinket to function correctly and consistently? Do you understand the process of how to do it?

Okay, so it appears you have not fully trained it, so that’s one thing; we cannot expect it to work 100% if you have not trained it 100%. Upon getting to “6” on Fire Shot you should be good to go … or at least better.
However, there do appear to be some bugs with this trinket.

That said, it does not appear your issue is related to those bugs, and it’s just a matter of having not trained it fully yet.
I’ll do it’s final training later today.
All and all if the way it’s working right now is by design it’s a very poor and confusing design that most average players will not benefit from.
It doesn’t seem right that even after five days of training it’s only going of once per entire dungeon run and having almost no benefit.
It’s a very unique item, though.
If it functioned like you want it to, there really wouldn’t be a need to train it, right?
