Today I got the word that despite an ergonomically correct setup, I’m having my first warning signs of an RSI. On balance, this shouldn’t surprise me, I’ve been playing WoW for 14.5 years now. I probably am getting my butt kicked by players who weren’t even born before I started doing these keypresses.
So it seems a hiatus is in order, and after that a relook of my control hardware. Has anyone got experience with a hardware or wow-legal hardware-software combination that can reduce top-row keypresses for a rotation? I already use POWA so I’m only casting abilities when they are up rather than mashing, and I also use a 15 button right hand mouse, but I guess I need to do more.
I’m thinking RSI’s are going to be more of a thing in this community as original players age up.
Being 52, originally having carpal tunnel like problems when playing the original Diablo (stupid bow character), the best thing I’ve found is to do things that work your hands in other ways.
Like it seems the best when I play something like Wow on the computer, then play something like “Monster Hunter World” (or something else) on the XBOX with the xbox one controller.
Meanwhile, just doing one of them all the time seems bad.
So it isn’t so much a certain different set up, rather you should probably just have a couple of different options to switch to.
Medical fields need to not reuse acronyms. The med school folks I knew all went into residency or emergency, so I read RSI and wondered what the hell you were doing on WoW that caused you to need a rapid sequence intubation o_0
I play wow with a controller. It was very hard to learn it but it is honestly just as good as good as keyboard now. I do since Legion.
Combine “ConsolePort” with " DynamicCam" add on, and you’re set.

Xbox 360 windows edition. I also use it for FF14. Playing MMORPG with controller is pretty enoyable.
The re-learning curve is a little test of patience but I do heroic raiding nowdays using a controller and do achieve epic parses.
I had a similar issue. I resolved it by taking more frequent breaks, buying a more ergonomic mouse and replacing my fancy mechanical keyboard with a good quality rubber done one. The biggest impact was replacing the keyboard. All mechanical keyboards provide far too much resistance these days.
