Real life suicide missions usually have a decent chance of survival, because no rational person voluntarily agrees to throw their lives away for a temporary gain. Even suicide bombers have to be mentally conditioned if not outright blackmailed into strapping a bomb to themselves.

Yeah if I recall correctly they didn’t give Imperial Japan Kamikaze pilots enough fuel to get back or even a life vest. So, ya could make it quick with a crash or either drown or die of exposure/thirst in the middle of the Pacific Ocean.
Typically people only do last stands when there isn’t a preferable alternative. This why having some merciless army that leaves no survivors isn’t the best idea. As if people know they’re dead either way, they’re more likely to go down swinging. Easier to just convince them to give up and surrender. Typically means less death on both sides.

I’d have the Scarlet Onslaught roll in to be the main antagonists of Tirisfal, Silverpine, Gilneas and Hillsbrad.
Figure we could say the Scourge Unleashed could’ve bolstered their numbers and now they’re here to purge the North of the inhuman corruption (Forsaken and Worgen). Forcing the two countries into an uneasy truce where subfactions of the undead and Worgen are still fighting in Alterac, the Dalaran Crater, etc. But officially they’re working together to fight the Scarlets.
The Gilneans would get Southshore and I’d also turn one of the Alliance camps in the WPL into at least a small Gilnean town like Surwich. Also eventually Preywood Village. I’d have a questline advancing in patches where Horde and Alliance players have to gradually push the Scarlets back to SFK (Scarlet Flame Keep, as it’s called now). And have that culminate in a Raid set in a redone Onslaught Keep which I’d make a giant cathedral not dissimilar to Castle Nathria.
The primary antagonist of the Plaguelands would be a now much more powerful Cult of the Damned. As they were able to seize control of throngs of unleashed Scourge they’ve taken back chunks of the Plaguelands but I’d have the Argents keep Hearthglen and also claim Tyr’s Hand as a refuge. I’d put both a Forsaken and Worgen embassy there as well. Plus a subplot about Voss or Calia trying to help the Scarlet Risen. The Forsaken will have also rebuilt all of Andorhal in their image by this time as well. The Worgen meanwhile were able to seize and do the same to Darrowshire.
Other than that I’d just include a lot more Revantusk Primals in the Cenarion Circle’s rehabilitation project. Because it’s weird the Forest Trolls never get anything to do but die.
