I have killed Kael three times for “Kael’s Vial.” To address the feedback I received on an in-game ticket: I don’t already have the item, my inventory has free space, I have the quest in my quest log, I solo kill him, and I always loot. The item doesn’t drop. I have the other item for this quest and it dropped without an issue.
