For the quest: “Dragul and the Hammer,” it is noted on wowhead and I have personally experienced WoW not recognizing the exit from Bitestone Enclave at first. I got to the questgiver, then had to circle back to the cave exit and walk around a bit for the quest tracker to pick up my exit.
This quest still isn’t fixed in 2025. It’s not very dramatic to relive a death scene 2 or 3 times to get a quest to script properly.
