I have never rolled onto a PvP realm until last night (new toon, not this one).
What is different about flagging myself for PvP on PvP realm from doing that on a PvE realm? AND, if I don’t flag myself, when exactly can I get attacked (or ganked)?

Kill or be killed? Go into a contested zone and you’re flagged. No exceptions.
Unlike in PvE realms, you are automatically flagged for PvP upon entering contested zones.
S’long as you didn’t flag yourself, it’ll drop off after 5m(?) after entering friendly territory and not engaging in PvP.
As for when you can be attacked, the answer is as soon as you’re flagged.

Ok that makes sense. Does my nameplate color change then to Yellow, and then back to Blue after I leave contested area (with 5 min delay)? And how many contested zones are there?
If the zone name is in green you’re in a safe zone and won’t be flagged, if you enter a yellow or red zone you’ll automatically be flagged.
And yeah, you’ll still be flagged for 5min after leaving a contested zone and then your name will return to blue.

Welcome to the PvP side! It has some frustrating moments but also so great PvP moments! Its also thrilling to quest next to an enemy and not know if they will attack.

You have Faction territory, and contested territory. Territory your faction owns means you can’t be attacked by the enemy faction if they stroll through, but your faction can attack them at will…they will be like a Yellow Mob.
Contested means no faction controls it, and both factions are auto flagged for PVP and can freely attack each other regardless of class, race, or level, or even gear. open, world PVP.

Just right from the start man as soon as you load into the starting zone just /pvp and stay flagged every waking moment you online. That how you pvp server man.

Last night… I killed a poor poor soul outside SFK…
My alliance group arrived at the instance entrance. A horde attacked us at the entrance. I assume he just wanted to poke and run back into the instance, but little did he know we had a fear. We feared him away, and killed him.
A buddy of his… a shaman… came out of the instance. He was unflagged.
He wanted to help his friend and started to rez…
He apparently had no idea he was going to be flagged for pvp if he rezed his friend xD. I even layed a trap right under him as he was rezing.
It was mean… but I had to do it…I even saw a horde guy standing next to him trying to warn him… but it was too late lol. I saw him spamming /say doing /no moments before the spell ended.

Pretty much if you are out of the starting areas its game on…however you will find some areas are not as bad as others. Enjoy it…No other way to play…look over that shoulder
Just to elaborate on something Blastology said, enemy players in your territory can attack pvp-enabled players.
Much like yellow mobs, attacking them turns them red.

Ah so even in a protected zone, if I am flagged PvP I am fair game. But if not flagged (edit: AND am not in a contested zone), opposition faction cannot attack me. But if I attack them first (edit: when blue), or otherwise help an ally (as the SFK story above says), then on goes my flag.
Sounds very adrenalin building.

You’re always flagged for PvP except in lower level zones.
Refreshing to see someone ask. Sometimes people just think “I like PvP, I’ll join a PvP server.” But that’s just not what a PvP server is about. Those guys get in and cry “But wait, these guys aren’t fighting fair! This isn’t real PvP!” If you worry about things being fair any percentage of the time you fight, PvP realms will likely not be for you.
It is, I love it. Leveling in Hillsbrad atm and there are nights when a horde of Horde have to band together to take out higher level Alliance.
Had an instance the other night where two Lv?? Hunters left me be, but a Mage 4 levels higher attacked me and paid the price. The hunters /cheered me after, guess they didn’t like him. XD
I’ve also had Lv?? Players splatter my innards everywhere just because they happened by me on their way to the next zone.
I guess sneaking through contested or opposition zones isn’t really a thing, then - except for rogues maybe - since the nameplates can be seen sometimes right through trees, etc
And we all know tanks can’t scamper much, lol.
On a Pvp server, if you enter a “contested” zone you can attack and be attacked by anyone with no exceptions. However, every races starting zone, plus the follow up zone (like durotar and the barrens, tirisfal glades and silverpine forest, dun morogh and loch modan) are safe zones for their respective faction. So for example, a horde player cannot be attacked in their safe zones (durotar, the barrens, mulgore, tirisfal glades, and silverpine forest) unless they attack an alliance member first, or flag themselves through other means. Same goes for the reverse, alliance members cannot be attacked by horde members in their safe zones unless they attack the horde player first.
So basically levels 1-20 are safe because you level in your safe zone, and then after that every zone is “contested” and its kill or be killed.
