There are eyepataches, from quests rewards throughout the game, and pirate hats can be purchased from the island expedition scenarios in BfA content. This hat came from one of them. Also, you can get the rest of my transmog from doing the Bloodsail Adirmal acheivement.
There is a good example of how it can combine the two.
Yes but you have to pick the hat OR eyepatch, not hate WITH eyepatch.

The blindfold still makes it look good, though.
It’s not bad, but many of us would like a hat with an eyepatch to look like a typical pirate.

Although, most pirates didn’t have eyepatches. This is even true for privateers.
I know, but those of us who want to look like the fiction depiction of a swashbuckling pirate with an eyepatch and pirate hat do want to be able to achieve that look.
