Probably rescued him in one of your manyRetrieve 20 lost souls from the maw/torghast quests.
If not, pay closer attention next time.

You guess correctly. But I don’t believe it was Bastion leveling, it was during the Kyrian 9.0 campaign IIRC.

Kind of. His soul essence becomes a part of Vol’jin, giving Vol’jin the power of the Loa of Kings. Vol’jin then goes to Ardenweld to be reborn in the seed cocoon.
The Kyrian are so pathetic, they willingly send him into the Maw because of “duty.”
Ben Howell probably got instantly obliviated by Kel’thuzad or something. Hopefully not, though.
In all likelihood, they probably scrapped Ben Howell as a mission table follower because they felt it undercut the emotional impact of Ben Howell’s soul being sucked into the Maw and never being seen again.
They’re never going to show or tell us of Ben Howell’s ultimate fate, of whether or not his soul survived being in the Maw or if he was obliviated.
I don’t know that I agree with them keeping Ben Howell’s fate ambiguous, but it is what it is.
