There has been a bug that has existed since Legion that causes certain abilities, spells, and other various effects to depict a metamorphosis demon of the opposite gender. I’ve experienced this while playing night elf male, so I don’t know if it happens to other types of DH"s.
Furthermore, the game itself treats you as if though your gender has changed.
Where I’ve observed this:

Blades Edge Arena. Being in metamorphosis while low on health will cause High King Maulgar to shout, “FINISH HER” when he should be saying, “FINISH HIM”.

Cutscenes. Entering a cutscene while in metamorphosis depicts me as a female demon.

Reflecting Prism. Using this on friends while in metamorphosis will work correctly most of the time. However, occasionally, if I’m trying to apply it on someone who already has an altered appearance, it will depict them as a female havoc demon despite their gender.

Grand Delusions. If in metamorphosis and this corruption procs, the Thing From Beyond will be depicted as a female havoc demon.

Horrific Visions. The madnesses of Dark Delusions and Split Personality will depict a female havoc demon if they occur while you’re in metamorphosis.

Kil’jaeden encounter. The Shadow Reflection ability will depict you as a female demon.

ON BETA. The ability Unbound Chaos summons a female version of your inner demon.

ON BETA. Entering metamorphosis as a Venthyr DH causes me to lose the title “Count %t” and earn the title “Countess %t”, and then vice versa when exiting metamorphosis.
Places where I have previously observed this bug that have been addressed:
- Legion artifact trait Inner Demons would spawn a female demon instead of a male one. This was addressed sometime before the end of Legion. On beta, this ability as a legendary power seems to be functioning properly.
- Ai’li’s Skymirror. If I had captured an image of myself in metamorphosis, casting Eye Beam would transform me into a female demon for the duration of the cast.
