Can we get any kind of official post on whether horrific visions are supposed to drop a piece of gear every time?
Considering they are gated by currency I assume they should (as well as that being the case on ptr).
Me and my wife have done atleast three with only the one piece of gear dropping for each of us.

I got a wrist for doing mine on my warrior… too bad I got a better wrist earlier on in the quest chain.

It took two days for people to get a response.
And no.
You only receive a piece of gear if you complete more than one objective. If all you do is kill Alleria/Thrall, than all you get is basic loot (pets, Essences, currency, etc.)
We did an extra objective just last night and got only an essence shard and cloak upgrade
ive completed 3 with an extra boss and got 0 pieces of loot. the one i did with just thrall was the only one that gave me loot, and it was 25 iLv lower than i was already wearing with terrible stats.

I got gear on the first Thrall kill. The three other times I didn’t. I never killed an extra boss.
I said gear, not loot.
My understanding is that this system is similar to Mythic+; once per week you can enter a vision and obtain a piece of gear. The more objectives you meet, the more ilvls are added to your gear.
After your first run of the week, you get loot: essences, pets, currency, etc…
That is the problem. Some people aren’t getting gear. Loot is going missing as well.
I did 4 runs (1 with a side area), didn’t get any gear though I did get a soulbound pet
I got a corrupted piece of gear from just killing thrall. But only on one run not any other runs. The understanding even on guides like wowhead was even just killing thrall would give Gear of 420ilvl every run
3 chested 4 runs across 2 characters, got no gear at all.
