Characters names-servers and what you ran on that specific character would be helpful. Especially the ones that ran Mythic + and Heroic raid bosses since those are fairly easy for us fellow players to validate.
Are you sure the world event rewards aren’t just old rewards? If you ever completed stuff but then never went and picked it up (even from weeks ago) then it’s still there. You have to claim all older rewards before you can get the most recent. Near the bottom of the screen, I believe, it should say that it was an older reward if so.

Indeed it does. I just had an alt claim her old Vault rewards.

Seems this is what was happening. It did not have the messages at the bottom of the window for me (maybe a conflict with an addon), so that was the root of my confusion.
