Good friend was having the issue of not being able to add/change spells on the action bars. He disabled the addon, FishingBuddy, and now everything is working properly

I had the exact problem. After skimming this discussion I disabled Bartender, which completely threw my UI into chaos. So I turned it back on and tried someone’s suggestion of holding Shift while I clicked and dragged. Worked like a charm.

except bar mods do the action better than the base UI… im using CT mods for that reason way more customizable and acurate without the clunk of the new baseline
