I completed the next step in the necrolord campaign upon reaching renown 5. Afterwards I attempted the achievement “Nine Afterlives” but Mayhem the cat is no longer at his location at the house of eyes necropolis.

Seconded this issue - but it’s not that he’s no longer there, it’s that he’s not present in this current phase. If you use your portal up to the large bone pylon right next to his location from the outside of the temple area, you can clearly see him sitting there. The second you jump down to actually click him, he fades away as you enter the phase. It’s seemingly impossible to work around this phasing issue; I’ve been trying for nearly 30m now to get to him before he fades or find some way to change back to the older phase where he is still visible, but nothing is working.
