So earlier last night I noticed nameplates disappeared on only one character - this one that I’m posting on. As I went through all my other characters, I saw none other were affected.
First I checked addons and the in game settings to see if I had accidentally disabled a feature on just this character. When that didn’t work, I disabled all my addons, because sometimes that does make a difference. When that didn’t work, I went ahead and did a full UI reset - deleted the Cache/Interface/WTF folders.
This character still has no nameplates showing. All of my other characters are fine. When I adjust the nameplate settings in game, they move around like it’s trying to register, but it still only shows the names, not the actual nameplates.
Here’s a SS of my in game names settings… maybe I’m overlooking something?

You can see in the bottom left, above the chat box, I have an enemy selected, and even then the nameplate doesn’t show up.
The weird thing is, it still shows the elite icon beside where the nameplate should be.

Any suggestions?
Press V or whatever keybind you have toggle enemy nameplates bound to.
If the design changes then nameplates are being toggled on/off and you have something else changing how they look.
Next time, prior to resetting your whole UI, try resetting your CVars to default (this is also catastrophic but slightly less so).

The less catastrophic version would be to install AdvancedInterfaceOptions, search its CVar Browser for nameplate related CVars and reset just those to default.

They’d already mentioned resetting their UI didn’t fix it (which it wouldn’t if it’s a CVar issue).
I wasn’t sure what to expect with that… so I was surprised. 
Ok so it’s fixed, but it was the strangest thing… In my interface it showed I had Alt-V bound to enemy nameplates, but when I used that keybind, nothing happened. The names flickered, but no nameplates showed up… but it still showed “Always Show Nameplates” and “Enemy Units” were checked, meaning they should show up.
I reset all my keybinds to default, logged out of the game and back in, and used the original default keybinds to toggle the nameplates and it worked just fine.
Not sure what happened there, Twilight Zone moment maybe… but in any event, thank you for the assistance.
