Not clear on what is the allowed nameplates for instances in hardcore. I have tried to give nameplates some indicators of what the debuffs are by using the things I used in other versions of WoW. As a healer, it’s especially important and with old persons eyes I find myself targeting a player and squinting. If I need to completely wipe out my addons to get some debuffs on nameplates, I will do that.
I do have my nameplates nice and how I want them outside of instances only.

I am confused as to what your problem is, there is no limitation/requirement about nameplates. However if your nameplates are changing when you go in an instance where they are different then outside of the instance, thats likely related to an add-on. Blizzards nameplates wont change when you enter an instance, you either turn them on or off.
Do you use any add-ons for nameplates, or UI add-ons? Knowing what UI/nameplate add-ons you use might help identify how to correct the configuration.
I have an addon for improved nameplates. It does not work in instances.
What is the actual add-on? most add-ons I am aware of don’t care if your in an instance or not.
Big Debuffs, Threat Plates, Plater. None of them work but only in instances. My hunch is that I had a my cataclysm classic folder install wonky and mixed up with this folder.
Thats likely, probably delete them, and reinstall them all, if you still have problems, disable them, then add them one at a time and see if one is causing the problem. Most people I know that use add-ons like that are all tanks, and they specifically need them for instances, so they should work.
Ignore everything I said, I saw a thread on the general forum, it looks like friendly nameplates have been disabled in instances, it appears to be related to how they could be used to make mythic content to easy.
Instead of nameplates, you might want to consider raid frames that you can customize with similar information. I know I use Grid2, which allows me display debuffs of different types. Grid can be a bit complex though so there are easier add-ons to use.
Cell is a unit frame addon that works really well with default settings. It shows buffs/debuffs/shields/status and if you don’t like the default set up, its really easy to change. You can set up click healing but i don’t, i just use it for info.
The similar addons already suggested are also good, just more fiddly to setup (personal opinion, you might like them better)
Unit frames are much better for tracking the info you need, they are static and don’t float out of view off screen or change size.

You might have the option enabled that changes your frames depending on your party size. Search under interface in the settings.
