The great vault is based on your top ten highest M+ keys for the M+ Slots. If you ran less than ten keys then imagine those slots are all filled with a key the same level as the lowest key you did run. So if you only run 2 +4 keystone’s in a week. Imagine you have ten slots where the first two slots are filled with a 4 and the other 8 slots have imaginary 4s in them.
Now at the end of the week the vault takes your top ten keys ran and checks slot 1, 4, and 10. If you have a real number in that slot you get loot corresponding to it. If you have an imaginary number there you don’t get loot. So if you run 4 keys you can onlly get two pieces of gear. And they are based on your highest key and 4th highest key.

It is still a stupid system. But blizzard likes to implement overly complicated stupid systems.
Mythic Dungeons : The item level of rewards in this category is determined by the lowest level of your top 1, 4, or 10 dungeons for that week. If you complete 1 Mythic +10 dungeon per week, your lowest dungeon is your top 1 dungeon, so you will receive an item appropriate to the item level of a Mythic +10 dungeon. If you complete 10 Mythic dungeons per week with the lowest being a Mythic +6 clear and your highest clear a Mythic +12, the rewards will be based on the lowest level of your top 10 clears, so you will get an item appropriate to the item level of your Mythic +6 clear. The item level increases when you complete higher-level Mythic dungeons.
The phrasing is stupid, I think it’s basically based on your highest, 4th highest, and 10th highest. So if you ran a 4, 3, 3, and 2, you will get 2 choices, one at ilvl of a M4 and another at ilvl of M2.
