Could be griefing, could be boosted and bad.
Also, no. IO is a measure of your beat 8 runs. It has no metric to decrease without deleting runs that you did, I don’t know if that’s even doable by the devs but it certainly shouldn’t be for failing a key.
I can think of 1000 ways to make you quit the key before I do if I really had to do that. There would also be zero way for the developers to tell if I did it on purpose, especially if I don’t write anything in chat. If there were leaver penalties, the griefing would be outrageous.

You probably actually couldn’t if this system was in effect, but I’m sure you could get yourself reported, by trying
play better and people will stop leaving your keys. not sure what else to tell you here.

When did this happen? what key, what dungeon?
Of course I could. I could pull extra mobs as a dps “by accident” I cannot press defensive as a tank and fall over. I could stop healing as a healer or I could just AFK completely run around in circles. How are any of those things reportable? Maybe I’m just terrible. Are you suggesting that it’s a reportable offense if somebody’s just not very good and they could get banned as well? Is that healthier for the community? Quite frankly I’m offended and find even the thought of such a thing to be reportable!
I’ve seen players in 15’s still accidentally do this. Don’t think so deep into bad play/mistakes. There isn’t always some underlying evil lurking in the darkness. Most people want to win believe it or not and don’t want to be the reason a key depletes. It happens

“the going gets tough” shoudl not mean that people who have higher IO than I do are making stupid mistakes over and over again.
Not timing keys is too punishing and a waste of time (literally) If you dont want people to leave, play better.
this was a 2700io priest, we all ran away they dropped it ontop of us twice, like hello, if you have 2700 io you shouldnt be chasing the group with breath that 1 shots
Playing devils advocate here, but did anyone ping a drop location for the breath? Was it discussed beforehand? Once your chosen for the frontal you have very limited time to make a decision if it’s unclear and different groups do different things with this mechanic. Minimal communication is required, even a simple ping goes a VERY long way in no comms pugs. Again, sounds like a panic move and not a grief. If it happened back to back it’s likely that priest was rattled from the first mistake and couldn’t recover mentally. These things happen, you learn from it (hopefully) and carry on to the next one
Also seen plenty of tanks position the boss so he’s still in the slime rather then pull him out so the entire group could just spread around.
Most people don’t like it when people just bail. Unfortunately, the fixes people propose are always worse than the bailing itself.
People don’t generally set out to wreck keys and they don’t generally leave if they are going well or if expectations are being met. I have found though that setting the correct expectations BEFORE you start the dungeon when you are putting the group together minimizes this issue.
