happened to me heaps of times. even just switching from 1 character to another and back. not a fun time.
Also just happened to me. Switched a Mists to Sanguine Depths and logged out yesterday without having used this key, but had done another dungeon after the switch with someone else’s key. Came back just now and it’s back to Mists.
I had this happen as well. I had a 15 wake key, that I got from failing a 16, then completed a gambit and rerolled it, got mists, now its back to wake the next day.
This has happened to my friend and I 3 times in the past week. Tonight we were going for the last two +20 keys we need for achievement (PF and DOS) and neither of us had a key for them so we pugged in to a Plaguefall and timed it. I changed my key from HOA and it turned into a DOS which was awesome. I logged off for a minute and when I came back my key had reverted back to HOA. My friend changed his key from streets and got SOA and decided to log off to check if his key changes and his did too. It went from a SOA to a ToP. This is getting really old. This thread is filled with people having the same issue and not a single blue post on it. Please fix this.

Happened to me today (july 10) changed my 20 halls to a streets before logging out for the night and the next day it was back to a halls. Hopefully the issue gets fixed soon.
pal and i just had this happen today. both of us switched our keys and logged. came back and back to originals
Happened with my keystone today. Changed yesterday, logged in today, completely different key then the one I received when I changed it.
Happened to my hunter this week. Had a 20 ToP, changed it to a 20 gambit, then it reverted back to a ToP.
