Every time I enter/leave an instance all my poisons are removed except for crippling poison as assassin.

This usually happens when you get into a rated match and apply your poisons while waiting for the countdown. Apply your poisons before you get into the queue, and they’ll stay on after you’re back out. I believe it’s been like this for multiple expansions now, and I think it has something to do with letting you choose your poisons for that specific rated match only.

This used to happen in timewalking as well but probably still does with most instances you enter.
