Was Questing in “The Deeper” cave in Kun Lai Summit. Came across a spear stuck in a rock, upon clicking the spear I was awarded an achievement and a loot window popped up containing the “Hozen warrior spear”. After Hovering over the item for about half a second the loot window closed and I never got my spear.
