As per the title. In Zereth Mortis, I encountered a Mezzonic Cache in the Fissured Breach at 43.6, 40.2. I tried to solve it myself, restarted the puzzle, left and re-interacted, reloaded UI, nothing was able to make it so I could click any of the squares on the top two rows, making the puzzle unsolvable.
