I feel like with the stigma already surrounding mental health, this would be a fine line to walk and as Rosenivy mentioned, I wouldn’t trust them with such an issue. While there’s the potential to reduce stigma and raise awareness, it could easily backfire. Given the general culture seen in this community, I could see people making a mockery of these issues… quickly.

Mental illness exists ingame. Going through the entire game you come across both young and old war veterans who had ptsd, they’re usually orcs.
Some goblins have obsessive/compulsive disorder, that fat prince Gallywix shows this.
Sylvanas was a sociopath til this expac, now that she’s talked Anduin into doing what she wants, she’s having misgivings.
Maybe she’s growing a soul, Idk.
So yeah, most are war related but this is a game of war so that makes sense.

Also, just thought of another one. In Silverwood, the BE starting zone there’s a quest where a male BE is having hallucinations, etc… he exhibits either schizophrenia or some other psychotic break.
Not saying they shouldn’t be.
This toon sufferers from depression brought on by loneliness due to lack of solo content. My psychiatrist in Darkshore recommends PVP therapy and daily conquest points.
The most frightening thing about mental illness is that you could be suffering and think everythong is normal. In fact thats likely how it starts

Sat speechless for weeks seems like an actual human response to escape from eternal torment. Anduin’s reaction seemed more unrealistic, when you unchain him and he just joins up with you as if you’ve just bumped into him outside the supermarket.
It was a tad jarring for those characters to seem to get over the ordeal just fine. It really makes the maw seem not that bad which is the exact opposite of how we should feel.

Objectively, no. None of characters are real or conscious and can’t be mentally ill. That’s not helpful though. I don’t post attention enough to lore to say whether or not it’s represented in WoW but from the stuff I have seen I haven’t seen any direct examples of the topic.
We see several examples of madness in WoW: some withered Nightborne go insane in an odd temporary limbo state between full feral withered and their old selves, the “mad” duke Theotar and the other Emberward inhabitants, Murozond in end times, etc…
