Background to fight. Thanos uses infinity gauntlet to open a portal into another universe, that universe is the Warcraft Universe. He ends up on Azeroth, Westfall, Pre first war. Medivh Senses his coming via massive power surge in the ley lines and goes to confront him.
Round 1 Medivh going all out with his guardian powers vs Thanos using the infinity gauntlet to full effect. Who wins?
Round 2 (if Medivh loses round 1) The portion of Sargeras in Medivh helps him fight Thanos, as he doesnt want to lose his puppet/doorway into Azeroth. Who wins?

If we’re talking about comic version Thanos, it’s not even a contest. Movie Thanos was a lot weaker by comparison.

Lets stick with Movie Thanos, since comic Thanos, if I’m not mistaken, cannot die, because the personification of Death will not accept his death.
(Though I dont know if that applies to him when he’s in a different universe.)
Medivh easily. The Infinity Stones only work in the universe they are from, they’re just shiny rocks in a different universe.

Even if infinity stones worked in the WoW universe, I don’t know how well they could be used against someone with the essence of a Titan in their body. We are talking about literal gods here.

the guardian power has limits… we saw this such as with Aegwynn, she struggled to fight just an Avatar of Sargeras (not even the real Sargeras)…
Thanos with the infinity gauntlet on the otherhand, can delete people from existence, turn back time, and warp reality. So if Thanos wanted to, all he would need to do is imagine that he’s the Guardian and it would become reality… So now you have Thanos with the power of a Guardian and the Infinity Gauntlet… GG
Well, in reality it would be Sargeras vs. Thanos, so I think the dude who cleaves planets in half wins. I don’t think he can snap Sargeras away if he couldn’t stop the Avengers from distracting him enough from snapping in Edngame.

it’s exactly how they work. the infinity gauntlet is literally as powerful as the imagination/will of it’s bearer. Also don’t forget that the “reality stone” is apart of the gauntlet, which has the power to reshape reality.
The Reality Stone allows its user to alter reality to what one wanted or break the laws of reality, logic and physics. Minor uses allowed one to resurrect the dead, distort reality around someone so it was incomprehensible, or other things that would normally be impossible. More powerful uses allowed one to create any type of alternate reality one wished. At its peak when backed by the other gems it allowed one to alter reality on a universal scale.
So with that stone alone… Medivh doesn’t stand a chance. Thanos can distort reality for medivh, take away his powers, and more.
In infinity war, Thanos was struggling vs Dr. Strange, even though Thanos had the reality stone. Why didnt he just take Dr. Strange’s powers away then?

Wouldn’t make for good comic books or movies if bad guys actually used logic.
Comic Thanos wins because the IG was significantly more powerful in the comics. Movie Thanos loses because he got his head knocked off at the beginning of Endgame by the weakest Avenger.
