A GM advised me to report one of these bugs on the bug report forum, so I will report them all.
- The Stormpike Perfection achievement does not trigger, but every other achievement does. All four Alliance bunkers were blue, all four Horde towers were destroyed (none of the eight were contested), Balinda was alive, and only then was Drek’Thar killed - but then no one on the Alliance team got the achievement. This seems to be at least two years old: Didn't Receive Credit for Alterac Valley Achievement
I am most annoyed by this bug and can provide screenshots of it upon request. It is very hard to get this achievement and it sucked to not receive it due to a bug.

The “Capture a Mine” quest inconsistently triggers. Sometimes it triggers when I am at the entrance of a mine and have just entered - sometimes it doesn’t trigger when I was the first to pull Masha Swiftcut / Keetar and did a lot of damage.

When a mine is captured, the NPCs for your faction do not always respawn. This seems to be a longstanding issue that happens more often when control of a mine changes frequently, and is complicated by the fact that the hostile NPCs are intended to remain there if they defeat the new arrivals. Recommended fix: When a mine changes control, loop through all NPC spawns for that faction. If the NPC belonging there is not present, spawn it. If it is already there, don’t spawn it.
-
Sometimes when a neutral mine is captured (Taskmaster Snivvle or Morloch) control of the mine will not change. The neutral boss will stay dead and the mine will be unclaimable.
-
When defending a contested Graveyard, the four flag defenders (Stormpike Defender / Frostwolf Guardian) do not always respawn. Same recommended fix as #3.
-
When a guard upgrade occurs, not all mobs are affected. I have often seen e.g., Stormpike Guardsman patrols after Seasoned Guardsmen have entered the battlefield. Recommended fix: Same fix as #3 and #5. Additionally, have units that are currently in combat remain in combat at their current strength. Add a check after an upgradeable mob exits combat to upgrade it if applicable. However, if the NPC that controls that mob’s respawn (e.g., Commander Duffy) has been killed, don’t trigger the upgrade and instead leave the mob alive at its current strength once it exits combat.

The Marshals that spawn after the Alliance destroys a Horde tower (e.g., Iceblood Tower) have half the maximum HP as the Warmasters that spawn after the Horde destroys an Alliance bunker. I remember this issue still being the case years ago.
As a final note, the scaling by level in Korrak’s Revenge is terrible. Level 20’s regularly get killed in one or two hits by level 80’s.
