What am I missing?
I’ve done it on 3 chars so far (to no avail) where I grab the letter by the mailbox in Legion Dalaran, I take it to the underbelly to turn in with Madam Cho (or whoever the black market lady is) and then . . . nothing. No breadcrumb shows up.
Wowhead says I should get mail to send me to the pet cemetery area to mail sort but I don’t get anything.
So is this a known issue? Is it an issue or player denseness? >__>
Note: This is the quesline to obtaining Katy Stampwhistle, the mobile mail toy.

How long have you waited for the mail to show up? I can’t remember if it was instant or took a little while to get sent out.
It’s been just over 4 hours on the first toon I tried on. I’ll keep checking their mail.
Hmm not sure then, I doubt it would take longer than the usual hour or so if there even was a wait.
I’d suggest posting in the bug forum as well, maybe someone else there might no something and if not at least it will be on the Devs radar more readily.
Ohh just as an asides of you’re just after a portable mailbox, there is an easier one to get in Dragonflight.
Edit: really only worth getting one or the other if the mailbox is your goal as this and Katy share a cool down.

I ran that questline not long ago.
After you give it to Madame Cho, you’re supposed to get a quest to go to the entrance pipe to the mail sorting room, which apparently is no longer given - probably a bug. But if you find the entrance, which is approximately behind the block that has the first aid office (near the ledges), you can click it and enter.
I did the quest up until the last one where you have to sort 30 pieces of mail. Or however many. I didn’t come close, and figured I’d do it when I felt very sharp and had lots of caffeine.
A week later the toy came in the mail.

I did it 2 years ago but I remember having to do 2 or 3 other quests that came in the mail before finally the sorting quest
