are you talking about the “spectral essence” trinket? If you are, you are able to get it by using “Krastov’s bag of horrors” atop a pile of bones at the end of instance, i believe its the room on the second floor in the middle. Once you see a ghost right click and there you go, you’ll get an untracked quest to do to make it.

I farmed heroic across 10 characters for three days until some random person whispered me and asked if I was farming it. They let me use theirs to start the quest, so I gave them 100k gold for their kindness.
Kind of insane that you need a super rare toy to even start this content.
