Interesting, post up that macro I'd like to see how its written; I have a good idea how they may have done that.
The Backstab / SS macro is really barbarically simple. It will always try to cast backstab, but if it cant it will then cast Sinster strike. That way if the target turns when you least expect it, you use SS instead.
