And adoption is not clear if that would work. He wouldn’t feel guilty about flirting with a commoner if social class meant nothing. So an orphan may not have the social class needed.
Anduin can still throw himself into a relationship or a fling while still being on the brink of mental collapse or in distress.
And if Anduin adopted an orphan, they would then be his blood heir, thus become a member of the nobility, and therefore eligable to take the throne after Anduin steps down or passes on.

They can but - if they want their audience to accept/pay for what they write - they need have consistency… and have created a society where there are commoners, and there are nobles and lords… which is has built in class based society values.

In what world is forcing or expecting a potentially gay king or lesbian queen to have a biological child, even for business reasons, not homophobic. We’ve already established that Anduin does not need a queen to have an heir.
